BOOKING TERMS AND CONDITIONS

To book your attendance at the 6th Annual ASTRA Awards please complete the Booking Form below.

Tables will be allocated on receipt of payment and entry to the 6th Annual ASTRA Awards is by Entrée Card.

If you are making a group booking, maximum seating per table is 10. If you are booking for less than 10, seats at your table will be allocated to other people attending.

All Entrée Cards will be forwarded by mail to the person nominated on the booking form. This person will be responsible for issuing the Entrée Cards for group bookings. ASTRA does not accept any responsibility for issuing Entrée Cards to anyone other than the person nominated on this form.

Bookings close on the 11^th April 2008* and all names of those attending will be required by that date.

The 6th Annual ASTRA Awards will be held at The Hordern Pavilion, Entertainment Quarter, Moore Park. Please ensure that you arrive by 6.45pm as this is a filmed event and late access will be restricted.

*Bookings may be made after this date subject to availability. All late bookings must be paid by credit card.

CANCELLATION POLICY
A full refund, less a 20% administration charge will be made for cancellations received in writing before the 1^st April 2008. For cancellations received in writing before the 13^th April 2008 a 50% charge will apply. No refunds are payable after the 13^th April 2008.

PRIVACY

Your personal information will be stored on a secure database and will not be disclosed to anyone other than the organisers of the 6th Annual ASTRA Awards.

ASTRA may also use and disclose this information for future marketing purposes.
